    SCHOOL PHYSICALS - (HEALTH APPRAISALS) Students in grades Pre-K / K, 1st, 3rd, 5th, 7th,
     
    9th, 11th, ALL new enterers/returnee's, students participating in sports & students applying for
     
    a working card are required to have a current physical within the last 12 months.  Physical exams are
     
    offered throughout the school year at no charge. Our Medical Director is Dr. Thayaparan Mathanakaran.  If
     
    you prefer that your child have a physical with their  own doctor, please contact your child's
     
    School Nurse and let them know, and your child will not be scheduled for a physical. You will
     
    need to provide a copy of your child's physical to the nurse as soon as possible, however. NO CHILD
     
    will be approved for sports participation without a current physical on file  with the School Nurse. If
     
    your School Nurse does not receive a copy of the required physical within 60 days of the start
     
    of the school year (unless you give your child's School Nurse an appointment date) your child
     
    will receive a school health appraisal with Dr. Mathanakaran per NYS Department of Education Law 
     
    903(3)(a) & 8 NYCRR 136.3 (c)(1)(iii).
